​Please only complete this form if your child is in YEAR 8 - born between 1 September 2011 (01/09/2010) and 31 August 2012 (31/08/11). Forms to be submitted no later than 7th February 2025.

​Please read this covering letter from the Service Lead before completing the consent form.

The Human papillomavirus (HPV) vaccine that protects against several types of cancer is being offered to your child at school. For full protection against HPV as of April 2023 only one injection is now required. Your child's school will let you know when the injection will be given.

The leaflet ‘Your guide to the HPV vaccination’ will give you all the information required to allow you to make an informed decision. Please discuss this with your son or daughter, then complete this online form before the vaccination is due. Information about the vaccinations will be put on your child’s health records.

Please ensure that you only submit the consent form once.

We will not vaccinate a child without a valid consent form.
