Eligibility This consent form should only be completed if your child is in Year 8 and was born between 1 September 2012 and 31 August 2013. School Vaccination Visits Manx Care’s School Vaccination Team has now visited all secondary schools to offer the HPV vaccine to Year 8 students who were registered in advance. If your child was not registered in time If your child is in Year 8 and you still wish for them to receive the HPV vaccine, you can complete the online consent form. When completing the form Select “catch up clinic” when asked which school your child attends. Once your form is submitted, the Vaccination Team will contact you directly to arrange an appointment.Before You Complete the Form Please read the covering letter from the vaccination team which contains important information in relation to the HPV vaccine (link below) before filling in the consent form for your child. Please read this covering letter from the Vaccination Team before completing the consent form.About the HPV VaccineThe Human Papillomavirus (HPV) vaccine helps protect against several types of cancer. It is being offered to your child through the school vaccination programme. Since April 2023, only one injection is required for full protection. For more details, please read the leaflet 'Your Guide to the HPV Vaccination' and discuss it with your child before completing the online consent form. Vaccination details will be added to your child’s health record.ImportantSubmit the consent form only once to avoid duplication. A child will not be vaccinated without a valid consent form. Your child’s school will inform you of the vaccination date.
